Assumption of risk

I understand that ________________ are contact activities involving physical exertion, strikes, throws, takedowns, joint locks, chokes and sparring. Despite instruction and safety measures, injuries can occur, including bruises, sprains, fractures, concussions and in rare cases serious injury. I take part voluntarily and accept these inherent risks.

Health declaration

I confirm that I am in good health and know of no medical reason that prevents me from training. I will tell the instructor about any injury, illness, pregnancy or condition that may affect my safety or that of others, and I will stop training if I feel unwell.

Club rules and conduct

I will follow the instructions of the coaches and the rules of ________________, train with control and respect for my partners, keep good hygiene (clean training clothes, short nails, no jewellery) and not train under the influence of alcohol or drugs. The club may refuse or end participation for unsafe behaviour.

Release of liability

To the fullest extent permitted by the law of ________________, I release ________________, its owners, coaches, staff and volunteers from liability for injury, loss or damage arising from my participation, except where caused by their gross negligence or wilful misconduct, or where such a release is not permitted by law. Nothing in this form limits rights that cannot be excluded by law.

First aid and medical treatment

In case of injury, I consent to first aid by club staff and to the club calling emergency services. I understand that any medical costs are my responsibility unless covered by insurance.

What a good waiver covers

  • Participant and emergency contact details.
  • Assumption of risk that names the activities: contact training, sparring, throws and submissions.
  • A health declaration so coaches know about relevant conditions.
  • The release itself, limited to what the law allows.
  • Consent for first aid and emergency treatment.
  • Guardian consent and signature for minors.
  • Separate, optional consent for photos and video.

Waivers do not replace insurance or good coaching

In many countries a waiver cannot exclude liability for negligence or personal injury, and consumer law may limit what a club can ask members to sign. A waiver mostly proves that the participant understood the risks. Keep public liability insurance, qualified coaches and safe mats – and have a local lawyer adapt the template.

Collect signatures without paper

Paper forms get lost, go out of date and are hard to find when you need them. MatSync lets members read and sign your current club documents in the app when they register, and keeps every signed version with its date.

Frequently asked questions

Is a martial arts waiver legally binding?

It depends on your country. In many places a waiver records that risks were explained but cannot exclude liability for negligence. Use this template as a starting point and have it reviewed locally.

Do minors need a separate form?

A parent or legal guardian must sign for anyone under the age of majority. Enable the guardian section to add their details and signature.

Should photo consent be part of the waiver?

Keep it as a separate yes/no choice. Under data protection rules such as the GDPR, consent for photos must be freely given and not a condition of joining.

Can members sign online?

Yes. Download this template for paper use, or upload your final version to MatSync so members sign it in the app during registration.

Run the academy, not the spreadsheet.

MatSync handles schedules, bookings, attendance, belt progress and payments in one place, with a member app for iOS and Android.
